Workwell agreement details

What is WorkWell and am I eligible?

  • The aim of WorkWell is to support disabled people and people with health conditions to start, stay or succeed in work. WorkWell can offer a range of support, and may include:

A personal assessment of your needs to understand what support might work best for your circumstances.Personalised, goal-based plans to address your physical, mental wellbeing, and social needs to help you return toand/or thrive in work.

Access and referral to local and community-based work and health support servicesSupport for employers to understand your needs and advice on how to provide workplace adjustments that supportyou (employers will only be contacted with your permission).

To be eligible you will be either:

  1. out-of-work and need health-related support to return to work or
  2. employed and either absent through sickness or struggling in the workplace due to a health condition.

You must also meet the following criteria:Your home address OR address of your GP/local Jobcentre Plus falls within the WorkWell Service.You are over 16.You have the right to work in the UK.Your WorkWell Service believes you have a health-related barrier to work.

  • What information will be collected about me, and who will it be shared with?
  • If you take part in WorkWell, we will pass your information to the Department for Work and Pensions (DWP).

This includes things like your name, address, telephone number, email address, and National Insurance Number as well as information on your employment, education and health situation.

DWP will use your information to confirm that you are eligible to take part in WorkWell, and to gather information from your records such as earnings and employment information. This information, along with information you tell us about your education and health, will only be used for the research purposes and nothing else – your right to claim benefits will not be affected.

DWP will share your information with IFF Research who are evaluating WorkWell on their behalf. IFF Research use this information to contact you to tell you more about the evaluation, and to invite you to take part in a survey and interview.

They will explain what taking part in the evaluation will involve when they contact you, and you will have a chance to ask questions. Taking part in the survey and interview will be completely voluntary.

Once the evaluation has been completed and your personal information is no longer needed, it will be securely deleted. The findings of the WorkWell evaluation will be published. However, evaluation findings will be anonymised and because your personal information will be deleted, it will not be possible to identify you in the findings. How will my information be protected Bristol City Council will be the Data Controller for its WorkWell service. DWP will be the Data Controller for the evaluation. This means that Bristol City Council and DWP will make sure that your personal information is kept safe and is handled in line with the Data Protection Act and the UK General Data Protection Regulation. What if I don’t want to take part anymore

WorkWell is completely voluntary, and you can stop taking part at any time without giving a reason. If you do stop taking part, you won’t be asked to provide any more information, but we will still use the information you have already provided to help with evaluation.

Your rights

You do not have to take part in the programme – it is voluntary and entirely up to you. Your right to claim benefits will not be affected whether you choose to take part or not. If you agree to take part but change your mind at a later stage, you have the right to withdraw and you don’t have to give a reason as to why.

You have the right to see any of the personal information that is stored about you. Work Well West Project Officer, Claire Edwards can give you a form to fill out to request to see this information. You can also make a request by emailing claire.edwards@workwellwest.org You can request that your personal information be amended if it is inaccurate or incomplete.

You have the right to report any concern about how your information is handled to the Programme Data Controllers by emailing:

The DWP Data Protection Officer, Dominic Hartley data.protectionofficer@dwp.gov.uk

How your information is used and stored

  1.  I have read and understood the WorkWell Participant Information Sheet.
  2. I have had a chance to think about the information in the WorkWell Participant Information Sheet, ask questions and have had these answered well.
  3. I understand that my participation is voluntary and that I am free to withdraw at any time. I know that my right to claim benefits will not be affected.
  4. I understand that my personal information will be shared with DWP.
  5. I understand that my personal information will be shared with IFF Research.
  6. I understand that my personal information will be stored on Work Well West’s Customer Relations Management System Open CRM
  7. I understand that the information collected about me for this programme will be kept confidential and used solely for service delivery and research purposes.
  8. I understand that I can see any of my personal information that is being stored about me.
  9. I understand that once the evaluation is completed and my personal information is no longer needed, it will be securely deleted.
  10. I understand that the findings of the evaluation will be published and will not contain my name or any information that can be used to identify me.
  11. I understand that if I withdraw from the programme, my information will continue to be used for research and analysis only.
